Junior Membership Parental Consent Form

All Junior members of Effingham Golf Club are required, as part of their membership application, to have a completed Parental consent form.

By completing and submitting the form below: 

Declaration

You consent to your child taking part in all the golfing activities that are organised under the auspices of Effingham Golf Club (EGC), including group and individual coaching, the use of the practice facilities, playing on the Effingham golf course and other golf courses and / or their practice facilities and you agree that this Parental Consent Form can be given to an official of any other Golf Club where the child may be representing EGC.

In the unlikely event of an accident or illness requiring emergency medical, hospital or dental treatment, you authorise EGC or its agents to sign on your behalf any written form of consent required by a hospital, medical or dental authority if a delay in obtaining your signature is considered inadvisable by the doctor, dentist or surgeon. (In any such eventuality every attempt would be made to contact you.)

You consent that your child can travel with a member of EGC staff or its agents to attend fixtures / events away from EGC and you also consent that your child's photograph may be taken and published by the Club in relevant sports websites, newspapers and magazines in relation to golf related activities and that you give your permission for your child to be videoed solely for golf coaching purposes. You confirm that you understand that this consent is for the duration of membership up to age 18 years and that EGC will be advised of any changes to the information provided including medical and parent / guardian details at which time EGC may require a new consent form to be completed.
